REMINDER! The FE-600 Assurance Final Expense application is now REQUIRED in all approved states.

The previous version of the application, the FE-500 will only be accepted in CA, CT, DC, DE, FL, ND & SD until further notice. CLICK HERE  to view map with required applications for each state. Visit to download the current FE-600 application and order bulk supplies for your business. Please discontinue use of the previous application in approved states, as it will no longer be accepted.

The new FE-600 application was designed to make selling our Assurance Final Expense an even easier experience for you and your clients.

The changes to the Assurance application include:

  • Updated, clearer language in the Medical Questions to help avoid confusion
  • Changes to the normal activities of daily living in Question 14
  • The removal of the previous Question 15 from the Medical Questions section
