Alignment Health introduces a NEW Heart & Diabetes C-SNP 005 plan in North Carolina to support year-round enrollment that ties into Alignment’s clinical model. Chronic Condition Special Needs (C-SNP) plans are a type of Medicare Advantage (MA) plan designed to meet the unique needs of people with one or more chronic conditions.

When is a Client Eligible For a C-SNP?

A Physician Confirmed Diagnosis in One of the Following Categories:

• Diabetes diagnosis
• Chronic Heart Failure diagnosis
• Cardiovascular Diagnosis of Cardiac Arrhythmias
• Coronary Artery Disease
• Peripheral Vascular Disease and/or Chronic Venous Thromboembolic Disorder
