Transact America Updates Payment Authorization Forms

Transact America is updating its payment authorization forms.

Starting June 16, 2024, the updated Payment Authorization form series will be available on the portals for new business and in force Transamerica Lifetime, Final Expense Solutions Portfolio, Transamerica Financial Foundation IUL (FFIUL), Transamerica Financial Choice IUL (FCIUL), Trendsetter Super Series, and Trendsetter LB products.

Updated Forms New Business and In Force

• Pay2022ALL for Final Expense and In Force Monumental products

• Pay2023TM for Term products

• Pay2023UL for Universal Life products

• Pay2022WL for Transamerica Whole Life 

Updated Third-Party Payer Forms

• ICC23 T-SU-MU3IC-0423 is the Premium Payor Supplement form required when the payer is different than the owner or insured (new business only)
• IPAYSUPP0524 is the Payment Supplement form required when the payer is different than the owner or insured (in force only)

There will be a 30-day grace period. To be considered in good order, the updated Payment Authorization forms will be required on July 16, 2024.
