Ascension's Footprint Shrinks for 2024

In an effort to better serve clients, beginning January 1, 2024 Ascension Personalized Care will discontinue coverage in certain counties in Indiana, Kansas, Tennessee, and Texas. While they are reducing their service area, they are fully committed to these markets.

Please note: clients who live in a county where APC is exiting will be transferred to a new health insurance plan by CMS. If they enrolled through the Health Insurance Marketplace they will need to visit healthcare.gov or call the Marketplace directly at 800-318-2596 to make any changes. They can also contact their broker with questions.
