Healthy Home Visits Program

Some Aetna CVS Health® clients are receiving emails from Signify Health encouraging them to schedule a Healthy Home Visit. If you receive this email read it thoroughly and reach out to your agent or Signify Health for more details.

The Healthy Home Visits program is offered at $0 to select Aetna CVS Health Individual and Family Plan members who may benefit most from additional care. It is not offered to all members.

Members in eligible states will receive a $100 gift card that they can redeem at thousands of retailers, including CVS, for completing a Healthy Home Visit appointment. The gift card will arrive by mail, 4-6 weeks after the visit.

Here are a few highlights about the program:

• Eligible members receive a 1-hour long visit with a licensed clinician in their home .
• Signify Health is part of CVS Healthspire™, providing in-home health evaluations to help address gaps in care and connect clients to community resources.
• If Signify Health offers video appointments using a phone or tablet
• Healthy Home Visits don’t replace the client’s relationship with their primary care physician (PCP)
